Pre-Vaccination Checklist
The following information is needed to assess whether a person can be vaccinated and which vaccines they may require. Please tell the immunisation provider if any of the following apply:
The person to be vaccinated:
- Is unwell today
- Has a disease which lowers immunity (e.g. leukaemia, cancer, HIV/AIDS) or is having treatment which lowers immunity (e.g. oral steroid medicines such as cortisone and perdnisolone, radiotheriapy, chemotherapy)
- Lives with someone who has a disease, which lowers immunity, or lives with someone who is having treatment, which lowers immunity.
- Has had a severe reaction following ANY VACCINE.
- Has ANY severe allergies (to anything)
- Has had a liver vaccine within the last month (this includes measles-mumps-rubella vaccine, oral polio vaccine, Varicella (chicken-pox) vaccine, yellow fever vaccine)
- Has had an injection of immunoglobulin, or a whole blood transfusion within the last 3 months
- Is pregnant
- Is living with someone who is not vaccinated
- Identifies as an Aboriginal or Torres Strait Islander person
Note: If you have any questions about this information or any other matter relating to vaccinations, please ask the immunisation provider before the vaccine is given.
Before the vaccination takes place, the immunisation provider will ask you:
- Did you understand the information provided to you about immunisation?
- Did you need more information to decide whether to proceed?
- Did you bring your vaccination record card with you?
It is important for you to receive a personal record of your injections. If you don't have a record card, ask your immunisation provider to give you one. Bring this record with you every time you come for injections. Make sure your immunisation provider records all vaccines on it.
