I need to manage incidents from an area
The management functions described on this page are currently available to the Local OHS Practice Expert in each division. Other staff can direct requests for change of supervisor or organisation, or provision of reports, to their Local OHS Practice Expert.

Can I get access to manage incidents for my division or department?
- The management functions described on this page are currently only available to the UOM EHS Incident Report Super User responsibility in Themis (refer: Themis responsibilities used for EHS incident reporting).
- UOM EHS Incident Report Super User responsibility is held by the Local OHS Practice Expert for each division.
- For other staff, please direct requests for change of supervisor or organisation, or provision of reports, to your Local OHS Practice Expert.
See what incidents have occurred
- To monitor open incidents via the EHS workbench, log in to Themis, go to your UOM EHS Incident Report Super User menu, and choose Review Status of Existing Incident Report.
- To review full details of an open incident, click the glasses icon. Navigate through each of the five pages of the report using the Next button.

- Note: this reference card shows staff self service users how to use the UOM Staff Self Service workbench. Functionality for super users is the same, except that you will go to the UOM EHS Incident Report Super User workbench instead.
- If you need to review information about more than one incident, you may prefer to run and export a report instead. Refer: Run/export reports.
Review and approve ('Close') supervisor actions
- Local OHS Practice Experts are responsible for using UOM EHS Incident Report Super User to confirm the appropriateness of the information that supervisors have entered in response to incidents. This is done by reviewing incident reports that have been Finalised by supervisors, and if satisfied, Closing them.
- incidents should only be Closed in conformance with the detailed protocol described in Themis Incident Reporting Procedure for Review and Closeout (login required). The following brief description of how to operate the Close function in Themis is provided as a convenience. It does not cover all your obligations when Closing an incident.
- to Close an incident in Themis, follow the following steps:
- log in to Themis, go to your UOM EHS Incident Report Super User menu, and choose Review Status of Existing Incident Report
- locate the incident in the table Incident Reports Awaiting Closure by EHS
- click the blue pencil icon
- the Person Details screen appears. Click Next to see the Witness Details screen (if there were witnesses), then Next to see the Incident Details screen. These screens contain information provided by the person who reported the incident, and most of the fields cannot be edited by you.
- now click Next to go to the Incident Response screen, then Next to go the Incident Classification and Investigation screen. These screens contain information provided by the supervisor, which you must review for appropriateness. After consulting the supervisor, you can edit these fields if desired.
- at the bottom of the Incident Classification and Investigation screen, there is a field Super User Comments. You must enter comments as described in the procedure Themis Incident Reporting Procedure for Review and Closeout (login required).
- at this point, note that this is the last opportunity for any further information to be added or changed, so make sure that both you and the supervisor are satisfied before proceeding.
- to finish, click Close. You will now need to complete some record-keeping tasks outside of Themis (refer Themis Incident Reporting Procedure for Review and Closeout [login required]).

Run/export reports
- Reports are available from the Discoverer Report item in the UOM EHS Incident Report Super User menu.
- A number of reports are available, but those relevant to Local OHS Practice Experts are:
- EHS Incident Mgt Rpt - Super User
- provides all information from all reports in the system that were created between specified dates (exception: the presence of witnesses and attachments is flagged by Yes or No. Review the incident via the workbench to see details of these items).
- EHS Incident Status Monitor - Sup Usr
- provides limited key information from all open incidents in the system at time of running
- EHS Incident Mgt Rpt - Super User

Change the supervisor
- This functionality is currently limited to users with UOM EHS Incident Report Super User. Other users, please refer to: My incident has the wrong supervisor
- Changing the supervisor is described in: Quick Reference Card: Changing the supervisor and organisation for an EHS Incident [.pdf]
- Please consult and inform all active stakeholders before changing the supervisor of an incident.
Change the organisation
- This functionality is limited to users with UOM EHS Incident Report Super User. Other users, please contact your Local OHS Practice Expert if seeking a change of organisation for your incident.
- Changing the organisation is described in: Quick Reference Card: Changing the supervisor and organisation for an EHS Incident [.pdf]
- Organisation should only be changed in consultation with the Local OHS Practice Expert for the proposed new organisation, and in conformance with the statement of principles available on the EHS intranet (login required). The high level principle governing Organisation is:
Organisation should reflect the division/department with which the injured/involved person has the most relevant relationship for the purposes of the incident.
