Report an incident, injury, illness, near miss or hazard
Emergency or immediate danger
If someone is in immediate danger or requires urgent medical assistance:
- Call 000.
- Then call University Security on 03 8344 6666.
- Provide first aid and take reasonable steps to protect yourself and others, if it is safe to do so.
For a serious or potentially notifiable incident, do not disturb the incident site except to assist an injured person, remove an immediate danger or prevent another incident.
Report through ERMS
Use the Enterprise Risk Management System (ERMS) to report:
- a work-related incident, injury or illness
- a near miss or dangerous occurrence
- a health and safety hazard
- a health and safety system failure
- property loss or damage associated with an incident.
Report an incident or hazard in ERMS
You will need your University username and password.
Tell your supervisor or the manager of the work area as soon as reasonably practicable. High-consequence incidents must be reported immediately. Other incidents must be reported within 24 hours of becoming aware of the incident, injury or illness.
If your supervisor is unavailable, involved in the matter or is not an appropriate recipient, contact your local Health and Safety Business Partner for advice.
If you cannot access ERMS
If you do not have a University username and password, or ERMS is unavailable:
- use the incident report form for an incident, injury, illness or near miss
- use the hazard report form for a hazard that has not resulted in an incident.
Provide the completed form to your supervisor or local Health and Safety Business Partner. The information must be entered into ERMS as soon as reasonably practicable.
Facilities and maintenance issues
Use Snap Send Solve to request rectification of facilities issues such as:
- cleaning or rubbish
- damaged furniture
- grounds or pathways
- lighting or power
- plumbing
- general maintenance.
If the issue caused an incident, injury, illness or near miss, also submit a report in ERMS.
If the issue presents an immediate danger, contact University Security on 03 8344 6666.
Serious or potentially notifiable incidents
You do not need to decide whether an incident is legally notifiable before reporting it.
Immediately tell your supervisor if the incident involved a death, serious injury or illness, hospital treatment, electric shock, fire, explosion, an uncontrolled substance release, a structural or plant failure, or another event that exposed someone to a serious and immediate risk.
Supervisors must immediately contact:
- the relevant Health and Safety Business Partner during business hours; or
- University Security on 03 8344 6666 for the on-call representative outside business hours.
Do not disturb the incident site unless necessary to assist an injured person, remove an immediate danger or prevent another incident.
Concerning workplace or student behaviour
Concerning behaviour may require both a health and safety report and a complaint or support referral.
If the behaviour caused, or could cause, physical or psychological harm, report the incident or hazard in ERMS.
For additional support or to make a complaint:
- staff can find out how to lodge a workplace behaviour complaint
- students and other members of the University community can contact the Safer Community Program.
If your supervisor is involved in the matter or is not an appropriate recipient, contact your local Health and Safety Business Partner for advice.
What happens after you submit a report?
The report will be assigned to a responsible person, usually the supervisor of the affected person or work area. They will review:
- any immediate action taken
- the level of risk
- whether an investigation is required
- actions needed to prevent recurrence.
You may be contacted if further information is required.
