Report incidents and hazards, and complete safety checklists

Choose the task you need to complete:

Incidents and hazards are reported in ERMS. Workplace inspections, checklists and workstation assessments are completed in UoM Riskware.

Emergency or immediate danger

If someone is in immediate danger or requires urgent medical assistance:

  1. Call 000.
  2. Then call University Security on 03 8344 6666.

Take reasonable steps to protect yourself and others. Do not delay emergency assistance to complete an online report.

Report an incident, injury, illness, near miss or hazard

Use the Enterprise Risk Management System (ERMS) to report an incident, work-related injury or illness, near miss, dangerous occurrence, health and safety hazard or health and safety system failure.

Complete an inspection, checklist or workstation assessment

Use UoM Riskware to:

  • complete workplace inspections and health and safety checklists
  • record findings and assign corrective actions
  • manage tasks arising from inspections or checklists
  • complete home or campus computer workstation assessments.

Not sure which pathway to use?

  • An incident, injury, illness or near miss occurred: report it in ERMS.
  • You identified a hazard outside a planned inspection: report it in ERMS.
  • You are completing a planned inspection, checklist or workstation assessment: use UoM Riskware.
  • You have been assigned an action from a UoM Riskware checklist: complete and close the task in UoM Riskware.

Facilities and maintenance issues

Use Snap Send Solve for cleaning, damaged furniture, grounds, lighting, power, plumbing, rubbish or general maintenance issues.

If the issue caused an incident, injury, illness or near miss, also report it in ERMS.
